Work Performed
Monitor daily cens usandassignment to assure all patients are assessedfor case managementn eedsincluding care coordination/transition anddischarge planning, consul tations, advocacy, education.Maintain effective communication with healt hcareteam members relatedto assessment findings, discharge planning need sand provider ordersneeded to arrange Homecare, Durable Medical Equipmen t,Transportation,Skilled Nursing or Acute Rehab Facility placement, Subs tance AbuseTreatment and outpatient follow-up.Maintain working knowledge ofspecific benefits and reimbursementguidelines, the discharge planningp rocess and applicable federal, stateand local regulations.
