The DRT(CT) technologist operates CT (Computed Tomography) scanners to acquire diagnostic images of patients, and support CT-guided procedures to enable the diagnosis and treatment of disease. Exam requests are screened for appropriateness and reviewed with a radiologist as needed. Exams are largely done by established protocol however some clinical presentations may require the technologist to tailor exams to meet the unique requirements or condition of the specific patient. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Certification: All applicants must be certified in general radiologic technology by the American Registry of Radiologic Technology, Radiography (ARRT) (R) and Advanced ARRT certification is required for assignments that include computed tomography (CT) Education: Completion of a full-time training course of at least 24 months duration (or the equivalent) in a post-high school diagnostic radiologic technology program. Evidenced by a certificate or an associate degree, accredited by the Joint Review Committee on Education in Radiologic Technology (JRCERT) or from schools accredited by the ARRT and certification by the ARRT in radiology. English Language Proficiency: DRTs must be proficient in spoken and written English. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Specialized Assignments: Computed Tomography (CT): This specialty modality requires specific knowledge of cross-sectional human anatomy and its application in spiral, and/or, multi-slice computer tomography, inclusive of 3-D reconstruction scans, including drainages, biopsies, and peripheral vascular examinations. The technologist requires specific knowledge and training in the location, appearance, and function of the various major and minor systems susceptible to radiological illumination; to interpret the examination request accurately; to understand the functioning and inter-relationship of the various organs; to use the methods and techniques which will identify organs appearing on the digital display monitor, or on film, and the various stages of the examination to judge the acceptability of the image and/or scan for diagnostic use and to emphasize the aspects of interest to the physician. [DRT assignments that include performance of independent duties in this subspecialty require advanced ARRT certification.] Grade Determinations: GS-9 DRT Experience: At least 1 year of experience equivalent GS-8 grade level directly related to the position being filled that demonstrates the clinical competencies described at that level. Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. DRTs at this grade level may be appointed to one of the following assignments: Diagnostic Radiologic Technologist. Employees at this level are fully functional as advanced DRT and carry out their assigned tasks independently. DRTs at this level may have varying assignments including special and complex imaging procedures beyond the full performance level, clinical instruction, and basic QM type duties within the program. Regardless of the nature of the specific assignment, the work must be of sufficient scope and complexity to meet the knowledge, skills, and abilities to perform at this level. The candidate must demonstrate all of the following technical KSAs and demonstrate the potential to acquire the assignment-specific KSAs designated by an asterisk (*): Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs *Ability to balance the needs of patients and staff while still performing complex scans and procedures. Knowledge of techniques for gathering relevant information from the medical record, significant others, and health care providers. *Ability to assess factors that may contraindicate the procedure. Knowledge of basic first aid and basic life support practices related to radiography. Knowledge of physical assessment, aseptic techniques, intravenous methods and techniques and universal precautions. Knowledge of pre-procedural, procedural, and post-procedural care of patients. Present unresolvable discrepancies or deficient indications to the Supervisor DRT(CT) or a radiologist.\nApply principles of customer service and whole health to promote a positive patient experience. Assist schedulers in making appointments that are timely, coordinated with other appointments when possible to reduce travel burden, and aligned with the overall care plan. Maintain awareness of imaging orders outside the immediate section to ensure completion of as many orders as feasible at the same visit. Inform patients and families in the waiting area when there are delays. Ensure prompt notification of scheduled patients for clinic cancellations, and ensure that new appointments are close to the original date, ideally within two weeks. Seek opportunity to assist patient in general ways that may be direct, such as helping them to a nearest restroom, or indirect, such as helping staff from other sections or services who are advocating for a patient.\nApply radiation safety practices, including the concepts of time, distance and shielding, and the ALARA principle of keeping radiation exposure to As Low as Reasonably Achievable. Apply knowledge of exposure factors in CT imaging and CT fluoroscopy to balance appropriate dosage control with image diagnostic quality, integrating knowledge of accumulated dosage and genetic effects related to patient age and sex, risks of tissue and organ injury, risks to reproductive organs and risks to fetus. Screen for pregnancy, checking serum beta HCG of female patients with any potential of being pregnant. Screen imaging history and pending orders to prevent redundant exams and unnecessary radiation. Discontinue duplicate orders. Maintain records of patient radiation exposure.\nEstablish venous access using a needle/sheath angio-catheter employing sterile technique. Assess IV access for adequacy to support high rates of injection. Load power injector using aseptic technique, applying safety checks to prevent air embolization. Administer contrast media under the direction of a radiologist or other privileged provider using a power injector or hand-held syringe. Monitor injection site during contrast injection to reduce the risk of extravasation, and to prevent high volume extravasation.\nAdministrative functions: register exams, status tracks completed exams using compliant CPT coding. Ensure completed exams are properly archived both in PACS and VISTA Imaging. Copy exams onto disc or other electronic media when ROI (Release of Information) is not available.\nStay current on new protocols, software changes, policies and procedures, and quality assurance requirements.
