The RN Case Manager will be responsible for providing Case Management services for high-risk Medicare Advantage members for the Independent Physician Association (IPA) within the state of Ohio, known as the Optum Care Network of Ohio (OCN-OH). The Case Manager works directly with the member, provider(s), facilities and other entities to ensure the most appropriate utilization of resources and coordination of care.
The Case Manager assesses members to create, implement and monitor an individualized care plan and evaluates the outcomes, ensuring options and services required to meet the member’s health needs are best utilized. The Case Manager provides advocacy, communication and resource management to achieve quality and cost-effective outcomes.
Hours: Monday - Friday 8am - 4:30pm.

Coordination of member’s care through the health care continuum and across various care settings
Collaborate with member, provider, facility and care team to assess, develop, implement and evaluate an ongoing plan of care to obtain best health outcomes
Provide health education, coaching and treatment decision support for members (including available community resources)
Ensure adherence to NCQA requirements for Complex Case Management
Act as resource to Network providers and OCN-OH team; functions in team/collaborative environment
Help patients make informed decisions and practice self-care by acting as their advocate regarding clinical status and treatment options
Develop effective working relations and cooperate with medical team throughout the case management process

Current, unrestricted RN license in the State of Ohio
2+ years of clinical/patient care experience
Solid knowledge of case management principles and healthcare management
Certification in Case Management (CCM)
1+ years of case management experience
Experience in outpatient care setting
Knowledge of high-risk adult disease state management
Intermediate level of proficiency using a PC in a Windows environment
