Under the supervision of the Clinic DON and the PH Collaborative Nurse performs general nursing duties relating to public health in Sheridan County at the Hoxie Medical Clinic. The employee in this position performs physical assessments, administers screening procedures, makes referrals to providers and screens individuals for proper health care placement. The nurse administers immunizations and prescriptions as ordered by providers.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Knowledge of current recommended immunization schedule for all patients.
Knowledge of current catch-up immunization schedule for patients’ age 4 months through 18 years of age.
Schedule well child exams.
Access patient immunization record through the EMR and WebIZ.
Educate patients and parents of HMC on the importance of immunizations.
Run monthly reports in WebIZ on patient 2 years of age and younger that are behind on needed childhood vaccines.
Contact parents of patients behind on childhood vaccine and schedule appropriate appointments.
Update and file “hard copy” of patients’ immunization records 0-18 years of age.
Review and update childhood immunization card with a note documenting all overdue as well as upcoming vaccines that are due.
Be available as a resource to clinical staff regarding immunization education and recommended vaccine schedules.
Regularly update WebIZ regarding patient’s activity status immediately upon notification of a child that is no longer an HMC patient.
Regularly communicate with interpreter staff regarding education information to be distributed to non-English speaking families relating to the importance of immunizations and health outcomes.
Monitor the VIS stock weekly and restock forms as needed.
Serves as a resource for clinical staff regarding education and instruction of the TB and cervical and breast cancer programs.
Assist the Community Educator RN with blood pressure clinics, medication set up, and diabetes classes.
Be a point of contact and subject matter expert for HMC patients on reportable diseases, including COVID-19, and assist the clinic staff with follow up calls, PPE management, etc.

Education and/or Experience
Current Registered Nurse licensure in good standing within the State of Kansas.
Previous experience in public health, nursing, health education, or health administration preferred.
BLS certification, within 1 month of start date, and bi-annually thereafter.
Participates in and completes 30 continuing education hours every 2 years as in accordance with the Kansas State Board of Nursing.
