The clinical and program activities assumed under this position are conducted as part of the Special Services Division, a unit of Mental Health Care Line (MHCL) at the Cincinnati VA Medical Center. This Senior Social worker will have part of their time dedicated to the Primary Care-Mental Health Integration (PC-MHI) team whose primary location is at the Cincinnati VA Medical Center. The primary job functions of the Special Services Senior Social worker will be to assist in the delivery of evidence based care for primary care patients identified with a variety of emotional and psychiatric problems. The incumbent will be responsible for completing appropriate assessments, making brief interventions (to include group and individual treatment modalities), monitoring patient progress, providing education and support, and ultimately influencing patient adherence to medical treatment recommendations by providing timely feedback to primary care physicians and collaboratively making appropriate care decisions. The senior social worker will meet current PCMHI certification and competency standards and support the VISN training/education/tracking requirements for certification of PCMHI providers at the Cincinnati VA. In addition to offering PCMHI services in primary care clinics the advance practice senior social worker will also support clinical programming in collaboration with coordinators in Family Services, Military Sexual Trauma, and the Intimate Partner Violence Assistance Program(IPVAP). A range of services may be provided to meet the unique needs of Veteran/ families including illness and psychosocial education, brief consultation, skills based and time limited groups, family and couples therapy. Clinical services are provided to Veterans diagnosed with complex medical and mental health issues. The Senior Social Worker performs work related to the review, monitoring, analysis, planning and assessment of the clinical programs the incumbent is serving. Responsible for supporting key project activities related to planning, development and implementation of initiatives. The incumbent will work as a member of the mental health care line interdisciplinary treatment team with regards to all aspects of the clinical program, bringing the Social Work perspective to the team as a whole. The incumbent will also provide education, consultation, and guidance to various other interdisciplinary teams within the Medical Center regarding PCMHI/MST/Family Services/IPVAP issues and treatment interventions. Incumbent performs specialized treatment associated with Whole Health Care, integrating physical and psychological health and functioning. Assessments: The social worker conducts brief assessments (typically 30min or less) with focus on the identified referral need, triages patient needs appropriately, and develops treatment plan based on assessment. Assessment is varied and determined based on presenting need and may include biopsychosocial assessments, cognitive and behavioral assessment, brief crisis assessment, and brief diagnostic assessments. Consultations: The incumbent offers consultation to interdisciplinary colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. The social worker provides consultation to both medical providers and Veterans to address psychosocial and behavioral health care needs of the Veteran and in collaboration with interdisciplinary treatment team, the Veteran, family, and significant others. Including collateral assignments working collaboratively within medical/surgical and mental health settings as assigned. Treatment Planning: The social worker is responsible for developing the treatment plan, setting achievable treatment goals with the Veteran/family and providing behavioral health care utilizing advanced clinical judgment to determine the appropriate treatment intervention for the Veteran and their loved ones.
