• Full Time Day Shift

• Performs evaluations and re-evaluations according to standards and recommended techniques.  Establishes, implements and adapts an individualized treatment plan of care including appropriate goal setting, treatment activities and frequency.  Discharges patients when maximum benefit has been achieved and/or according to standards of practice and payer guidelines.
• Documents accurately, completely, and timely with legible handwriting.  Uses measurable and objective terminology following standards of practice, insurance guidelines and department/ hospital policy and procedures.
• Charges patient accurately, timely and legibly according to guidelines of the CPT code format, price file, and following insurance guidelines.
• Utilizes knowledge of growth and development for assigned patient age population. Evaluates for age-appropriate behavior, motor skills and physiological norms of the patient. Interprets data relative to patient age.  Modifies approach and communication skills appropriate to the patient served, which may include: Neonate/Infant (birth < 1), Pediatric (1 < 12 yrs), Adolescent (12 < 18 yrs), Adult (18 < 65 yrs) and Geriatric (65 + yrs).   
• Identifies the need for patient/caregiver education and utilizes each opportunity for teaching on a planned or spontaneous basis.  Documents education and understanding as appropriate. 

• Bachelorâ™s degree in Physical Therapy and five or more years of experience as a physical therapist  OR  Masterâ™s or Doctoral degree in Physical Therapy and one or more years of experience as a physical therapist.
• Current valid State of Florida license as a Physical Therapist
• Board Certification from the American Physical Therapy Association (APTA)
• Basic Life Support (BLS) certification

Provides direct and indirect services to specific age groups in the acute care, inpatient and/or outpatient rehabilitation settings. Utilizes process improvement in achieving the goals of the Rehabilitation & Sports Medicine Department. May supervise other physical therapists, physical therapy assistants, volunteers, technical aides and interns. Facilitates development of staff clinical competency Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
