Job Summary: Assist in the planning, directing, and coordination of medicine and health services in long term care and assisted living facilities. Responsible for working with residents in evaluating residents' social needs, easing the transition to ghe facility and planning and organization of soicial activities for residents and thier families. Recognizing resident needs which may include transfers to appropriate facilities, working with facility team and family members. Assist resident in maintaining quality of life in most appropriate setting.
Job Qualifications: Minimal 18 years of age with high school diploma or GED. Prefer college classes in sociology and or psychology but not mandatory. Familiar with long term care or assisted living facility. Ability to read and write well and document appropriate findings and assessments.
